How to Register

The doctors welcome new patients who live within our practice area.

Everyone is welcome in general practice. Anyone can register without needing proof of ID, address or immigration status.

Check you live within the boundary area

To be eligible to register at the practice, you must live within the practice catchment area.

Complete a New Patient Registration Form

If you live within the boundary area, please complete our new patient registration

When you register, it's helpful to have your NHS number. You can use the NHS website to find your NHS number.

Register at our GP Practice

Alternatively, you can ask the receptionist for a registration form and questionnaire.

Your Named GP

As of the 1st April 2015, practices are required to provide ALL their patients with a named GP.

It may not always be possible to see your named GP; all clinicians have access to your medical records.

Your Medical Notes

Assuming you have been previously registered with a GP in England, upon registering with this practice, your electronic record will, where possible, be transferred automatically from your previous practice through the use of an NHS system called GP2GP.
